1. In the form, go to File > Template Properties.
2. Check Add a File-Cabinet to form files created with this template. This should only be a problem with multiple-page forms.
3. Make sure the fill field is defined correctly.

   For example: If you have defined a fill field, make sure that you didn't forget to set the field in the text layer as a barcode.
