SQL Server 2008 R2 Database Table Records as Merge Input Data

We have a SQL Server Database (Test18Modified) that holds a table (LTR_Correspondence). This table has all of our Correspondence Letter records.

I would like to use the iForms Server to automatically trigger and merge this table with our iForms Designer Form and then output these to pdf files.

In my opinion this should be possible. I must be missing something.

I have attached the log entries and DataSource setup.
